Douglas Middle School strives to protect each student from possible injury while engaging in Physical Education. The guidelines identified below have been established in order to protect the student and others from injury and /or illness. Students and their parents should recognize that conditioning, nutrition, proper technique, safety procedures, and proper clothing and footwear are important aspects of this Physical Education program. Each participant is expected to follow the directions of the Physical Education Instructor.
Guidelines are as follows:
1. Make certain that you wear proper clothing and footwear.
2. Advise the instructor if you are ill or have any prolonged
symptoms of illness. (Should be on record and verified by a physician's
statement)
3. Advise the instructor if you have been injured. Due to the
number of students in class, this is strongly emphasized.
4. Engaged in warm-up activities prior to strenuous participation.
5. Be alert at all times of the activities that surround you while
engaged in the activity.
Parents and Students:
1. Parents and Students are required to read all information issued in class pertaining to the whole Physical Education Program. The following information has been issued to all students:

The above information has been explained to me and I understand the list of rules and procedures. I also understand the necessity of using the proper techniques while participating in the Physical Education Program.
